The 55-year-old pop star did her best to defy a foot injury - a bruised bone sustained while dancing in high heels over a week ago - as she hobbled to dinner at Matsuhisa in Beverly Hills on Wednesday.
Though the cane made her appear older, she tried to keep a youthful image in edgy clothes as she leaned on a female friend and a cane for support while leaving the sushi restaurant.
The eye-catching ensemble featured baggy black leather trousers and a cleavage-baring, low-cut black camisole top that her patterned scarf did little to conceal.
A black trench coat and spiked boots helped to give the star a rapper-cool look and she might have pulled it off if not for the walking stick.
Gingerly Madonna inched her way across the car park, no doubt thankful to have such a tall, sturdy pal - in high heels too - to assist her.
Madonna has been dealing with the injury more than a week now.
It happened during a night of dancing in New York City following her return from a ski trip in Gstaad, Switzerland.
'She bruised a bone in her foot from dancing in her high heels,' her publicist told The New York Post's Page Six last week. 'She'll be fine and off crutches in a week.'
Earlier on Wednesday Madonna and her 26-year-old companion, Timor Steffens, hit the gym together for a class in Los Angeles.
While Timor was dressed all in black, including sunglasses, Madonna chose an Adidas polka dot jacket, a black cap with one white star and Love Pink logo trackpants.
Madonna's has long been known for her love of working out - in fact it has outlasted two marriages and plenty more shorter-term relationships.
It is, arguably, Madonna's greatest love of all and one which, in true Madonna fashion she turned into a moneyspinner.
Her Hard Candy brand of fitness studios are now littered throughout the world with the new opening, in Toronto, scheduled for February 11.
As part of the opening festivities, Madonna is scheduled to lead her signature Addicted to Sweat cardio-based dance class with the Hard Candy Fitness Performance Team.
In addition to a selected group of Toronto club members, Hard Candy Fitness clubs from around the world will hold auditions to send members to work out with Madonna live at the event.
With the addition of the Toronto location, Madonna's gyms are pumping hard in eight cities - Berlin, Moscow, Rome, Santiago, Sydney, St Petersburg and Mexico City.
It was only on Tuesday night that the Hung Up singer was pictured for the first time on American soil with boyfriend Timor.
And in another desperate bid to turn back the clock, the singer dressed in a torn-up street-style outfit which looked more suited to someone half her age.
The couple have only been spotted only a few times since it was revealed the iconic pop star had found herself a new younger dancer.
The star first debuted Timor - who is just a year older than her last boy toy, dancer Brahim Zaibat - during a New Year's Eve bash in Europe.
The new couple headed to a sushi restaurant in Beverly Hills, California, dressing from the same colour palette.
